Heat tolerance testing and the return to duty decision: A two-year case cohort analysis

Abstract

Heat tolerance testing (HTT) in those who have previously had exertional heat sickness (EHI) may help them assess their risk and return to duty or exercise. However, nothing is known regarding HTT's predictive validity, especially for EHI recurrence. Our investigation aims to demonstrate HTT's predictive validity in EHI recurrence, as well as its utility as a diagnostic tool in exertional heat stroke (EHS).

Methods: Participants with prior EHS were recruited for the study by physician referral and identified as heat tolerant or intolerant after completing demographics and an HTT. Participants were further classified as having single/simple (SS) or recurrent/complex (RC) EHI based on a two-year retrospective record review. We determined the positive (PPV) and negative predictive values (NPV) of HTT.
